2024 Health Plan Participation Form 1095-C Now Available in Workday

Employees who were enrolled in a university health plan in 2024 are now able to review and print their 1095-C form in Workday. Similar to recent Workday enhancements that have introduced electronic W-2s, employees were previously offered the option to receive a mailed copy of the form. 

The 1095-C form details monthly enrollment in the university's health plan for the prior calendar year.  The form is not required for filing individual federal or state of Arkansas tax returns, and the university still reports the information electronically to the IRS. 

How to Access the 1095-C Form

Use the Menu button in the upper left corner of the Workday home screen to navigate to the Benefits and Pay application. 

  1. Expand the Benefits section of the app and click on ACA Forms
  2. Click the View/Print button to generate a PDF file. 
  3. Click Notify Me Later
  4. When the form is generated, users will receive a notification with the link to the PDF file.
